This time around, due to time constraints, I am forced to streamline the collection process for the booklet data. So I created an interactive form that can be easily filled in and returned by simply clicking a button right on the form! We are focusing the booklet on the music this time, and NOT including artist bios. We thought that you all probably had bios floating around the web in various places already.
The form has fields for the Track Title, Artist Name, and a large field for you to type a description of the track. Nothing specific, just something about the music. What inspired it, how it inspires you or how you hope it affects listeners, production notes…whatever you want to say about your track.
Also, for a little fun, there's going to be some aleatoric bling on the booklet pages. A field is provided for you to enter a single alpha-numeric character. This will be input into the Artist Glyph Generator. We will also assign a random fractal image to each page based on the order in which the forms are received.
Note: This form is proven to work correctly when filled in using Adobe Reader (v7.0 or later) or Acrobat. Mac Preview will definitely not work. If you use a different PDF reader, the form MAY work. If you do not see a confirmation message after clicking on the Submit button, it very likely did not work. There is a link at the bottom of this post to get Adobe Reader for free. If you are completely Adobephobic, please PM me and we'll work something out.
